지도교수변경 신청서

Advisor Change Request Form

이 서식은 신청자가 작성하여 (변경 전)지도교수, (변경 후)지도교수, 그리고 선임주임교수님의 서명을 받아 교학처로 제출해야하는 서식입니다.

This form needs to be written by the applicant and needs to be approved by the current and new advisor as well as the head professor of GS TESOL. Once all filled out, please turn it into the admin office.
